Membership: Program Affiliation Process
Step 1: Complete the agreement form with administration
Step 2: Get a membership roster (template) from the school counselor. It should include the following data.

Column C: Email (email validation)

Column D: Grade (6, 7, 8, 9, 10, 11, 12, Collegiate Freshman, Collegiate Sophomore, Collegiate Junior, Collegiate Senior)

Column E: Gender (Female, Male, Not Selected)
Step 3: Upload both of the above files to this form.
